What to Expect After You Sign Up
Once you've completed your checkout process. You will receive your "Welcome Email" with instructions on how to follow the simple steps below.
Choose a General Dentist from our Provider Network.
Once you have selected a General Dentist, call their office and set up an appointment. Make sure to let them know that you are a member of Avenue Dental Plan so they are ready for you when you arrive.
Upon arrival, you will be required to show the receptionist a form of government identification to verify your Avenue Dental Plan membership.
Once the receptionist has verified your membership, you are ready to see your dentist!
Your dentist will complete your exam and give you a customized treatment recommendation. Your dentist will likely give you a print out of the recommended treatment options along with the fees associated. *Please note you are to make all payments for any treatment received directly to the dentist at the time of treatment.
