General

It’s like a Costco or Amazon membership, where you pay an annual fee to get reduced rates for dental services. These dental services include but are not limited to: whitenings, cleanings, crowns, veneers, routine check-ups for all ages, implants, dentures and more.

Dental Savings Plans vs Insurance Plans.

Dental Savings Plans:

  • No annual spending caps - use our plan as much as you would like
  • No waiting periods - use your benefits right away
  • No deductibles
  • No pre-authorizations required - get started right away
  • Big savings on preventive care, restorative care, and cosmetic dentistry
  • Directly available to individuals, families, seniors, college students, businesses, and other groups

Insurance Plans:

  • Most insurances have a maximum spending allowance per year.
  • Most insurances make their clients wait 6 months before they can use certain benefits.
  • Most insurances have hidden clauses and many exclusions to treatment
  • Most insurances have high co-pays and deductibles
  • Most insurance have limitations on cosmetic and orthodontic procedures
  • Veneers, crowns, and whitening are not usually covered with insurance, but with Avenue Dental Plans you get these at a reduced rate

The plan is less costly than many insurances. Insurances typically have higher monthly premiums compared to our annual and monthly membership plan fees. Many insurances also require co-payments and have deductibles in addition to their monthly premiums. Insurances also have yearly caps.

Avenue Dental Plan puts your dental care as its priority. There are no limits on how often you can visit your dental provider and how many times you can use your plan.

As soon as your payment is processed, you will be eligible to use this plan. We will send you a welcome email with your identification information and instructions on how to start using your membership.

Anyone can join! We offer plans for individuals, families, and groups. Please email us for information on group discounts, for groups of 10 or more.

You must be at least 18 years old to purchase an Avenue Dental Plan.

A.

    Avenue Dental Plans automatically renew either monthly or annually.

  • If you paid by credit or debit card, we charge your card based on your current plan and payment method. If you plan on renewing, you do not have to a thing.
  • If you prefer to cancel, please cancel your plan through your member portal or call us at
    1-888-764-5304 at least 30 days prior to your renewal date.

Customer Service Questions

We recommend that you consult with your dental provider about the dental services you are about to receive. If there are questions about discounts or estimates, please call Customer Service at 1-888-764-5304.

A.

Prior to the end of your Avenue Dental Plan contract, we’ll give you the opportunity to automatically renew your coverage for the following year, so you’ll have continuous coverage.

A.

Avenue Dental Plan membership fees are fully refundable within 30 days upon receipt of your application. After 30 days, you are entitled to a prorated refund based on how much time is left on your membership. Please note your one-time sign up fee is non-refundable. All refund requests need to be made by calling us at 1-888-764-5304.

This depends on what type of membership plan you have selected. If you choose a Single Member Plan, you are the only one who can use your plan for discounts. If you purchase a Member +1 plan then you and an additional member can use the plan. If you purchase a Family Plan then you and up to 5 additional members may use the plan. *Please keep in mind members can be added at any time up to the capacity of your plan however they may only be substituted once a year on your anniversary date. *Your anniversary date is one year from the date you become eligible for the plan.

To update payment information log into your member portal under “My Account”—> “Recent Orders” —> “View” —> Under Related Subscriptions —> “View” —> “Change Payment”

To add additional members: Call us at 1-888-764-5304 or email us at info@avenuedentalplan.com. *Please note additional members can be added at any time however they cannot be replaced until your annual anniversary date (this date is one year from the date you signed up)*

No. Purchasing two plans in the same office at the same time will not provide additional savings. The plans cannot be coordinated.

No. The dentist is under no obligation to provide discounted services to you before you become a member.


Getting Started

A.

Signing up is simple. Simply visit our Plan Pricing to sign up online and begin saving immediately.

We like to make this process as easy as possible for you. All you need to present is a form of government identification (ie. driver’s license) for yourself and all eligible members of your plan over the age of 18.

With Avenue Dental Plan, as soon as your payment is processed you will receive a confirmation email with instructions on getting started. Processing time may vary depending on factors such as payment amount and number of members. If you need expedited processing, please call us at 1-888-SMILE-04 and we will do our best to accommodate you, additional fees may apply.

At this time, we do not accept membership fees via US Postal Mail. We will accept payment on our website via credit or debit card only. Please select your plan and payment options here.


Discounts

All general dentists in the network have the same fee schedule however specialists have their own fee schedules and prices may vary.

We recommend that you mention you are a member of Avenue Dental Plan when you call to make your first appointment with your dentist. This will help save time during your first visit.

Payments are made directly to the dentist by you at the time of service.

Network

Avenue Dental Plan has a network of hand-selected dentists.Visit our FIND A DENTIST page. You can use the network map to help find the dentist closest to you.

Avenue Dental Plan has a network of specialists. Visit our website www.avenuedentalplan.com and click on FIND A DENTIST to help find the specialist you need.

Sometimes treatment plan fees don’t always look the same when printed. If you have any questions about how much you are being charged, please consult with your dentist’s treatment coordinator. If you any additional questions, please contact us.

Our network is constantly expanding. Your dentist may apply to become a participating provider right on our website by clicking on Become A Provider. If they have any questions or concerns they can email us or call us directly.

In order to receive discounts on dental services members must see a dentist or specialist within the Avenue Dental Plan network.


Troubleshooting

Please contact us at 1-888-764-5304. A customer service representative will be happy to update your information over the telephone.

Go to the Member Portal and click on “Lost My Password”. Enter your username or email address. You will receive a link to create a new password via email. If you do not receive a link or have additional trouble logging in, call us at 1-888-SMILE-04 or email us at info@avenuedentalplan.com
