General Questions

Please login into your Provider Portal. Click on Member Verification. Use the patient’s government identification to verify their name and date of birth are correct. Enter their name and date of birth in the following format:
firstname_lastname_MM/DD/YYYY in the search bar. For more details on how to search, please reference the step-by-step instructions on how to verify membership in Provider Web Portal.

Please make sure member’s are still active in their membership each time they visit your office.

Once you login to your Provider Web Portal. Click on Fee Schedules and select the appropriate fee schedule.

Avenue Dental Plan members receive free x-rays as part of their membership. If the fee schedule states $0 then please charge them $0 for that specific treatment. Please let us know if you have any questions.

Once you login to your Provider Web Portal and click on Fee Schedules, you have access to fee schedules for all specialists.

Please visit Find a Dentist page to see which other doctors are in our network and where you can refer patients to.

With Avenue Dental Plan patients are not required to see a general dentist before seeing a specialist. The necessity of having a referral is at the discretion of the provider.

Please make sure that patient’s are given a printed treatment plan with options and appropriate fees in accordance to the proposed Fee Schedule.
